Here is a checklist of the documents required for a Philippines tourist visa:
- A passport that is valid for at least six months after the date of entry into the Philippines that has two blank pages in it for the Philippines tourist visa and any other official stamps. It is a good idea to have photocopies of the ID pages as well as any previous visas for the Philippines.
- A completed Philippines Visa Application Form. This can be picked up at an Embassy or Consulate or downloaded and printed from the government website.

- Although not one of the requirements for entering Philippines, it is recommended that you take out comprehensive health insurance and travel insurance is always a good idea too.
